P2H Monthly Update: December 2013

Pathways to Health Alliance Groups

In the Tulsa community, there are agencies working in many program areas to impact health improvement. P2H has six Alliance Groups representing different sectors of work in the community: Healthy Kids, Healthy Places, Healthy Choices, Healthy Aging, Healthy Worksites, and Access to Care. Each group has two co-chairs that sit on various committees and attend multiple meetings relevant to their sector of work. Beginning this month, the co-chairs from all six groups met for a monthly meeting to enhance information sharing and cross-sector collaboration. These meetings will continue so that P2H can be better connected to community health improvement activities. Information shared will be reported in future monthly updates.

CHIP-ing Away: Join Us

Is your agency working to make an impact in one of the top six community health concerns highlighted in the Community Health Improvement Plan (CHIP)? We want to hear from you! Please take our information gathering survey here if you haven’t done so yet. Please fill out the survey for each unique program your agency has working toward community health improvement.

We will highlight the programs as Priority Champions on our social media pages (facebook and twitter) as well as in the annual CHIP update.
