Pathways to Health Block Parties

P2H has brought together 22 community partners  to interact with 470 community residents in three regions of Tulsa County! Through our block parties we are able to connect residents to the agencies working in their neighborhoods to improve their quality of life.  Many thanks to the agencies who have participated and the schools and park that hosted this Fall. We look forward to three additional block parties in early 2014. Email lchildress@tulsa-health.org if you would like to be notified of upcoming community  engagement activities.

CHIP-ing Away: Join Us

Is your agency working to make an impact in one of the top 6 community health concerns highlighted in the Community Health Improvement Plan (CHIP)? We want to know what programs are making a difference. Please take our information gathering survey here if you haven’t done so yet. Please fill out the survey for each program your agency has working toward community health improvement.

We will highlight the programs as Priority Champions on our social media pages (facebook and twitter) as well as in the annual updates of the CHIP.
